We collect information you choose to provide through private-client consultation requests, life-insurance qualification requests, appointment booking, and independent-contractor applications. This can include contact details, location, age, height, weight, general health information, general financial ranges, insurance goals, licensing status, experience, availability, and written responses. We also collect basic technical information needed to operate and protect the sites.
The One Standard Life qualification form is designed for a limited general profile, not a formal carrier application. Do not submit Social Security numbers, bank or account numbers, passwords, tax documents, medical records, or government identification through these forms. A carrier or other appropriately authorized provider may use a separate secure process if additional information is required.
We use submitted information to respond to requests, prepare for consultations, compare potentially relevant insurance options, reserve appointment capacity, review agency applicants, maintain records, prevent abuse, improve our process, and meet applicable legal or regulatory obligations.
Information may be disclosed to service providers that operate the sites, communications, scheduling, recordkeeping, and security, and to appropriately licensed or authorized professionals and entities when needed to respond to your request. We do not sell personal information submitted through these forms.
By submitting a form, you authorize contact about that request using the information provided. Message and data rates may apply to text messages. You may ask us to stop non-required communications, but operational or legally required messages may still be sent.
We retain information only as long as reasonably necessary for the purposes described, recordkeeping, dispute prevention, and applicable requirements. We use reasonable safeguards, but no online system can be guaranteed completely secure.
You may request access to, correction of, or deletion of information you submitted, subject to identity verification and records we are required or permitted to keep. Privacy rights vary by state.
